The Manager of Social Services responsibility is to provide overall direction and delivery of Social Services in Healthcare and Assisted Living at Hamilton Grove. To plan, organize, develop, and direct the overall operation of the Social Services Department in accordance with established policies and procedures. To assure that the medically related, emotional and social needs of the resident are met/maintained on an individual basis.

Duties include:

  • Complete resident's admission paperwork accurately and in a timely manner.
  • Maintain effective working relationships with medical and other health related facilities.
  • Develop, implement and maintain on-going quality assurance for the Social Service Dept.
  • Participate in quality improvement issues related to Social Services, meeting established goals.
  • Review and develop a plan of correction for social service deficiencies noted during survey inspections and provide a written copy of such to the Vice President of Operations.
  • Develop working relationships and effective communication with team members, residents, families and guests.
  • Interview residents, or family members, as necessary, to obtain social history.
  • Develop care plans and comprehensive assessments of the social service needs of residents are accurate and timely.
  • Maintain a productive working relationship with other department supervisors and coordinate social services to assure that daily requirements can be performed without interruption.
  • Inspect all social service functions weekly to assure that quality control measures are maintained.
  • Serve as referral source to residents and families in accessing community resources. Refer resident/family to appropriate social service agencies when the facility cannot meet resident needs.
  • Regularly review and evaluate departmental functions and make recommendations to team members.
  • Must be available for weekend coverage in the facility on a scheduled basis to ensure residents, visitors, and families are afforded access to Management staff.

Position Requirements:
A. Education & Experience

Indiana Health Facility Operational Standards require that in facilities of 120 beds or less, the Social Services Manager meet one of the following qualifications:

(1) Indiana board certification in social work under IC 25-23.6-5-1 with at least one (1) year experience in a health care setting working directly with individuals.

(2) A bachelor's or advanced degree, or both, in social work or a degree in the human services fields, including, but not limited to: (A) sociology; (B) special education; (C) rehabilitation counseling; (D) psychology; and (E) gerontology; and one (1) year of supervised social service experience under the supervision of a qualified social worker in a health care setting working directly with individuals.

(3) A high school diploma or its equivalent who has satisfactorily completed, or will complete within six (6) months, a 48-hour social service course approved by the division.

B. Skills and Requirements

  • Familiarity with health care programs and an understanding of the Hamilton Grove continuum of care.
  • Data entry skills.
  • Ability to use computer, typewriter, copy machine, and other standard office equipment.
  • Ability to show tact, patience, cheerfulness and enthusiasm with personnel, residents, family members, visitors, government agencies/personnel and the general public.
  • Independence, leadership ability, willingness to work well with and supervise other personnel.

Transportation Driver will drive bus or other vehicles for activities scheduled outside the facility, including but not limited to shopping trips, Diner’s Club, and doctor’s appointments.

  • Responsible for all residents when on an outing.
  • Assist residents on and off the bus or other vehicle and carry packages to the buildings as needed.
  • Clean the inside of the bus or other vehicles after each trip, check for left packages and debris. Clean the bus on a regular basis.
  • Responsible for routine maintenance checks and reporting any problems encountered to the Maintenance Department.
  • Complete all trip reports.
  • Other Duties as assigned.

Job Requirements/Qualifications:

  • Not For Hire Endorsement on Driver's License required or CDL and a knowledgeable understanding of Indiana law as it pertains to public transportation.
  • High school diploma or its equivalent preferred.
  • Previous experience in bus or truck driving preferred.
  • Ability to communicate by written and verbal word.
  • Ability to make necessary repairs in case of emergency in maintaining the vehicles.
  • Special interest in driving and in the aging population.

Gina Martin

Employee Benevolence Fund

Gina Martin believes in paying it forward. She received help when she needed it, and now she helps others whenever she can. Gina began working for Greencroft Goshen five years ago. During her tenure, she experienced some personal problems of her own, including financial hardships. Her manager helped her apply for a gift from the Employee Benevolence Fund. Each campus has its own fund, managed by the Greencroft Communities Foundation, and the process to apply is straightforward. To be eligible, an employee must have worked at their campus at least one year. They must be in good standing with no disciplinary actions or excessive absences. “I was so grateful,” Gina said. “It helped me climb out of a hole.”
